Individual Assess, Plan, Do, Review (I-APDR)

The Individual – Assess, Plan, Do, Review (I-APDR)

From September 2023, educational settings have access to the new Individual-Assess, Plan Do, Review (I-APDR) paperwork to record their SEND process for individual children and young people.

This does not replace the My Support Plan (MSP) but can be used instead of it if schools prefer.

Both the I-APDR and the MSP are non-statutory, schools can use equivalent in-school processes if these clearly exemplify a need that is beyond the scope of Quality First Teaching to meet, and which requires additional and/or different provision. In-school documentation must also show evidence of embedded provision and review.

You can find all of the relevant templates and guidance for the I-APDR below.

This videos talks you through how to use the I-APDR

  1. During induction the SENDCo will complete the initial I-APDR for the pupil coming in.
  2. In primary schools, the teacher should be responsible for completing the I-APDR throughout the year supported by the SENDCo. It is important that, where possible, the class support is included in this process. In the secondary phase it is important to consider who will be responsible for completing the I-APDR throughout the year and how the views of all subject teachers can be incorporated.
  3. It should be competed at the beginning of each term and reviewed towards end of term (three times a year)
  4. Review is completed with SENDCo with the teacher responsible It can also be reviewed through the parent/carer consultations. Pupil voice should be sought termly
  5. Utilise information from external/internal assessment processes
  6. Highlight interventions that are ongoing and ensure that you are clear what they are, what outcome they are meeting and the impact that it is having.
  7. Consider the best way to indicate any changes made through the review process: strike through, different coloured fonts etc.
  8. Always date the document in the ‘Review’ section.
  9. Be clear about whose responsibility it is to ensure that the support staff have access to the file and time to go through it.
  10. Be mindful of GDPR when considering how the file will be stored and shared. All addresses, phone numbers and CP info should remain secure within online systems.

Schools will find it helpful to refer to the LA’s Inclusive High Quality Teaching Audit Tool and Graduated Approach Toolkits to support this:

We believe that many schools will find the I-APDR easier to use than the MSP.   Detailed I-APDRs can be used to support requests for statutory assessment, the SENDACT team are familiar with the new paperwork and are confident that it supports the EHCP process.

The Regulation Plan is provided to support schools, only when it is needed. Similarly, the Making Action Plans Personal (MAPP) can support schools with person-centred planning but is not mandatory.

The Inclusion Support Offer (ISO) team are available to support schools wishing to implement the new paperwork
