Transitions between year groups, key stages and study programs, as well as other significant changes, can be a source of distress and anxiety for children and young people with SEND. Such transitions and changes are planned for in advance with the child and young person and their family.
Due to this, Kirklees has produced a protocol for schools to follow when young people with SEND are transitioning between settings and phases of education. This document gives advice on transition planning to ensure that those with SEND are well supported.
