Community nursing
Children's Community Nursing provides some aspects of nursing care to children and young people in Kirklees from birth up to 19th birthday.

In North Kirklees (WF12-17, BD19, BD12 - Batley, Cleckheaton, Dewsbury, Liversedge, Heckmondwike, Ravensthorpe and Mirfield) this service is provided by Locala
In South Kirklees (Huddersfield post codes) this service is provided by CHFT
Children's Community nursing supports children and young people with the following (this list is not exhaustive & each referral will be dealt with on an individual basis):
- Home oxygen therapy
- Oncology (cancer) and some blood related disorders
- Wound care e.g. post-operative wounds, pressure wounds
- Some aspects of epilepsy
- Children with long term conditions
- Gastrostomy & Nasogastric tubes
- End of life care for children with a nursing need who have life limiting/life threatening conditions
- Support with invasive/non-invasive ventilation
- Support families with injections
- Training and technical assistance with equipment
- Bowel & bladder dysfunction
- Sleep studies at a consultant request
What we do:
We provide some aspects of nursing care to children and young people in North Kirklees (WF12-17, BD19, BD12 - Batley, Cleckheaton, Dewsbury, Liversedge, Heckmondwike, Ravensthorpe, Mirfield or any child with a North Kirklees GP.) from birth up to 19th birthday.
A referral needs to be made online where possible or via Community Health & Social Care Hub. Please see link and contact details below.
Depending on the nature of the referral we will either see children in their own home or at an agreed place such as a health centre or the Child Development Centre at Dewsbury and District Hospital.
How to access this service
Children can be referred by a paediatrician, staff at the hospital, a GP, health professional
Once a referral has been accepted someone from the team will contact you for some more information & to arrange a visit/appointment.
If we ask you to come to a specified venue for your appointment these are all wheelchair accessible.
Frequency: The team work Monday to Friday 8am – 5pm. We do not cover Weekends or bank holidays.

Depending on the nature of the referral we will either see children in their own home or at an agreed place such as a school/Nursery or hospital setting
Children can be referred by a paediatrician, staff at the hospital, health professional.
A referral can be made by completing our referral form and submitting it to our generic email box childrenscommunity@cht.nhs.uk
Once a referral has been accepted someone from the team will contact you for some more information & to arrange a visit/appointment.
Frequency: The team work Monday to Sunday 8.30 am – 4.30pm and we also cover bank holidays.
