Community Plus can help you think about what changes you want to make and support you in achieve your goals, putting you at the heart of the conversation. We start with a 'Good Life Conversation' that looks at your strengths and helps create achievable goals in what matters the most to you.

Community Plus can support you by connecting you to:

• Local groups and activities
• Relevant information, advice and support services
• People in your community who are in a similar situation
