what is the DSR? The Dynamic Support Register is for anybody who has been diagnosed with a learning disability and/or autism, who are currently at risk of being admitted to a mental health hospital,placement breakdown or contact with the criminal justice system.
How to access the DSR? We can accept referrals from a person or their health and social care team. To refer in, please contact dsr.ckw@swyt.nhs.uk and we will send out a referral and consent from. Once we have received the consent and referral forms, we will contact the referrer to gather more information before taking it to our next DSR meeting to discuss as a MDT. Following an outcome from the DSR, we will contact the referrer to confirm if the person meets our criteria or we will signpost to other suitable services if the person does not meet our criteria. To be able to access the DSR we must have consent from the person. If the person does not have capacity, then the best interest section of the consent from would need to be completed by a health and social care professional. If a person declines to DSR support this will not impact their support, it will just mean the person would not be discussed with a MDT approach at the DSR.
