What we do 
  • Develop individual health / behaviour plans.
  • Promote the use of health action plans so they are recognised and used by all services.
  • Mental capacity / best interest assessments, where appropriate
  • Offer support with access to mainstream health services.
  • Occupational therapy support / advice and skills assessment to enable people to achieve activities of daily living independently.
  • Physiotherapy assessments to promote mobility, posture, function and independence.
  • Speech and language assessments and interventions to support communication and eating and drinking.
  • Psychiatric medical reviews.•
  • Psychology assessments, support and therapy.•
  • Learning disability assessments (diagnostic assessments).•
  • Work directly with individuals, carers and family.•
  • Provide intensive support for people with behavioural issues.•
  • Dietetic assessments and recommendations on what to eat and drink for improved health and wellbeing

We work together with other services including GPs, district nursing teams, adult social care, providers and carers, benefits, housing and tenancy support, specific support groups, education providers, dentists and other health care professionals to offer the best support.

How to access what we do

Referral criteria


  • The person must have a GP in the Kirklees area
  • The person must have a diagnosis of a learning disability unless they are referred for a learning disability assessment.
  • The person must have complex health needs due to their learning disability, which cannot be met by mainstream primary or secondary health care services when reasonable adjustments are made.
  • Referrals can be made by people with learning disabilities, carers and / or professionals.
