All about us

We will visit you in hospitalor at home and will listen and try to understand what’s working for you and what’s not. We will support you and your family to find solutions to problems and we will work alongside you and your family to get the right support. We will try to make sure your stay in hospital isn’t longer than it needs to be and we will support you to leave hospital when the time comes. We can then help you to find support near to where you live.
We will come with you to meetings about your care and support (with your consent) and will talk to other workers
in schools, health teams and other services to make sure you’re getting the best support.

Who is this service for?

Our service is for autistic young people and young people with learning disabilities.

  • Who are in a specialist hospital or might be at risk of going intoa specialist hospital because of mental health problems or might be at risk of going intoa specialist hospital because ofmental health problems.

  • Who are moving betweenchildren’s and adult services.

  • whose support from CAMHS is coming to an end aged up to 25 and from West Yorkshire.

How to access what we do

The needs of young people are discussed at regular DSR meetings and introductions to the service are then made by social workers, care co-ordinators or other professionals involved in the young person’s care and support.  We also welcome self-referrals if the young person meets the criteria.
