How to access what we do

We work in partnership with families and carers and other agencies. Referrals can be made to the service by local hospitals, paediatricians, other children's community nursing teams and healthcare professionals, GPs, Specialist Tertiary Centres, Social Services & Education.

Once a referral has been accepted someone from the team will contact you for some more information and to arrange a visit/appointment.  Depending on the nature of the referral we will either see children in their own home or at an agreed place such as a health centre or the Child Development Centre at Dewsbury and District Hospital.

You can also self-refer or be referred by friend or family member. Tel: 0300 3045555
